2. Enter your name as it appears on your card.
   
3. Insert Dr. Smith's License Number PSY21711 in the space for the License
   or Tax I.D. Number of Recipient.
   
4. Complete the form, then click on the Submit Payment button.
   
5. Print yourself a copy of the receipt that will appear on screen.
